Post Acute Partners is a New York-based owner-operator of post acute healthcare facilities.
Our mission is to be the premiere provider along the entire post acute continuum, from pediatrics to seniors, from assisted living to skilled nursing to long term acute care.
Strategically, we look to acquire geographically concentrated portfolios with great local reputations.
Operationally, resident care comes first – we work with our predecessors, our staff, state regulators, referral sources, and local communities to deliver excellence every day. When we acquire an organization, we aim to nurture the local character of these portfolios while introducing state-of-the-art operational techniques and technology.
What’s New?
In December 2010, we acquired Scallop Shell Nursing & Rehabilitation Center, a 72 bed skilled nursing facility in South Kingstown, Rhode Island. Scallop Shell operates under the name, Scallop Shell Nursing & Rehabilitation, a Post Acute Partners company.
In February 2011, we announced acquisition of a portfolio of five pediatric specialty care facilities and one assisted living facility for adults in Pennsylvania. This business operates under the name Pediatric Specialty Care, a Post Acute Partners company.
In April 2011, we announced the pending acquisition of a portfolio of 16 skilled nursing, assisted living, and independent living facilities from ElderWood Senior Care of Buffalo, NY. Located in Western New York, these locations will continue to operate under the ElderWood Senior Care name.
